Collected Personal Data:

Demographic information including name, address, telephone number, email address, GP details and emergency contact details will be harvested from enquiry forms, health screening forms and consent forms.    Health screening information, medication history and physiotherapy assessment and treatment details  will be recorded for each physiotherapy intervention.  

Data is harvested from contact forms, emails,client  interview/assessment and messages.  

Purpose of collecting data:

Data collected is used for communication in response to enquiries and services offered.  

Physiotherapists registered with the Health Professionals Counmcil (HCPC) are legally bound to keep accurate records of any patient intervention or interaction securely.  Retention schedules vary according to the type of record but, in general, for those with capacity it is usually: Eight years from the date of last treatment for adult records. Eight years after their 18 birthday or until 25 years of age for children/maternity records.

Email addresses for customer enquiries will be kept securely for 1 year for communication on services for marketing purposes.
